ACOM Aneurysm - Case 2
Ruptured Anterior Communicating Artery
History & Physical
- 63-year-old right-handed gentleman who experienced severe headache followed later by temporary episode of loss of consciousness. This was evaluated at an outside hospital with a CT scan, which reportedly was negative and his symptoms were attributed to dehydration. However, on the CT scan, an abnormality was identified and the patient was followed by a cerebral angiography of the brain, which was positive for an intracranial aneurysm.
- His neurological examination was non focal. He, however, had register of 3/3 and recall of 1/3 in five minutes.
Imaging
Cerebral angiography of the patient’s brain shows an anterior communicating artery aneurysm.
Surgical Procedure
- He underwent a supraorbital skull based craniotomy with micro-clipping of this aneurysm utilizing intraoperative neurophysiological monitoring.
Post-op Imaging
Post=op cerebral angiography shows complete treatment of this aneurysm.
Post-op Course
- He was discharged from the hospital with no complications and returned to normal function and remains free of any aneurysm.
