Case Presentation:

Cervical Stenosis - Case 1


History & Physical

  • 78 year-old right handed man presented with progressive loss of function of the lower extremities with loss of gait and balance, progressive loss of manual dexterity and weakness and numbness of hands, with numbness in the hands and feet.

  • His neurological examination showed weakness of grips (4/5), positive Hoffmann, positive Babinsky, and complete loss of sensation to pinprick in both the upper and lower extremities.

MRI scan of patient's cervical spine (neck) shows severe stenosis (narrowing) at C3-C4, C4-C5, and C5-C6.

 



Surgical Procedure



  • After a comprehensive work-up and eliminating other potential contributing causes of his disease, and an appropriate period of conservative measures, he underwent anterior cervical discectomy and instrumented fusion (ACDF) at C34, C45 and C56.

 



Post-op Course



  • He did well postoperatively with immediate improvement of his function and was discharged home.
